This case was referred since the buccal swelling was. not subsiding. The dentist who had treated prescribed antibiotics but that did not help.
When the case came to our clinic, i diagnosed on the radiograph that there is a perforation due to over enlargement of the post space. That caused a lesion in the furcation.
The treatment plan was to remove the fibre post and seal the perforation with MTA. As soon as the core filling was removed, pus oozed out. The fibre post was removed with ultrasonics. The entire canal including the perforation was filled with MTA.
A GIC cover was given over the MTA. Core was completed. A six month follow up showed complete resolving of the lesion.
